Terms and Conditions for Health Coaching Services

Welcome to Primal Vitality, where you will receive personalized health coaching from Carrie Hayes. Please read these Terms and Conditions carefully before engaging in our services. By purchasing, accessing, or using our services, you agree to abide by these terms.
1. Services Offered
Carrie Hayes, as a certified health coach, provides coaching services aimed at improving your overall health and wellbeing. Services may include, but are not limited to, personalized goal plans, guidance on nutrition, exercise programs, lifestyle changes, and motivational support. It is essential to understand that health coaching is not a substitute for medical treatment.
2. Client Responsibilities
Personal Commitment: Clients are expected to actively participate in the coaching process and adhere to the agreed-upon plans and recommendations.
Disclosure: Clients must provide accurate and complete information regarding health history, medications, and any other relevant information.
Communication: Clients should communicate openly with Carrie regarding their progress, challenges, and any concerns.
3. Payment Terms
Clients are required to pay for services in advance or as agreed upon in the coaching package.
Accepted payment methods will be specified at the time of service agreement.
All payments are non-refundable unless specified otherwise in these terms.
4. Session Scheduling and Cancellations
Sessions will be scheduled according to mutual availability and will take place either in-person or virtually as agreed.
Cancellations must be made at least [24/48 hours] in advance. Failure to provide adequate notice may result in a cancellation fee.
5. Confidentiality
All information shared during coaching sessions will be kept confidential, in accordance with HIPAA regulations, except as required by law. However, clients should be aware that complete confidentiality cannot be guaranteed in group sessions.
Limitation of Liability
To the fullest extent permitted by law, Primal Vitality and Carrie Hayes shall not be liable for any direct, indirect, incidental, or consequential damages resulting from the use or inability to use the coaching services.
8. Termination of Services
Carrie Hayes reserves the right to terminate coaching services at her discretion if a client is deemed not to be adhering to the terms, is disrespectful, or if the coach feels they cannot adequately serve the client's needs.
9. Modifications to Terms
Primal Vitality reserves the right to modify these Terms and Conditions at any time. Clients will be notified of any significant changes, and continued use of the services will constitute acceptance of the new terms.
10. Governing Law
These terms shall be governed by and construed in accordance with the laws of Ohio, USA. Any disputes arising from these terms will be subject to the exclusive jurisdiction of the courts located in Ohio, USA
